Sexual Health Inventory

The following is a simple questionnaire to help you assess your general sexual health. Your interest in sex, ability to become aroused and incidence of pain during sex are indicators of sexual function and can help you reflect on the quality of your sex life. Obviously problems cannot be diagnosed by one questionnaire, but it might help you think about your sexual situation in a thoughtful way, and flag problems if you are having them:

  3. In the past 4 weeks when you have attempted intercourse, how successful have you been?
  4. 1We achieved painless penetration almost every time.
    2We achieved penetration every time but it hurt.
    3We could achieve full penetration some of the time but it hurt.
    4I was able to achieve only partial penetration or penetration for a short time because of the pain.
    5I was unable to achieve penetration.

  5. When I have sex with my partner I get turned on:

    2Usually
    3Sometimes
    4Hardly ever

  7. Over the past 4 weeks, how would you rate your orgasms?

  8. During the last month how often were you in the mood to have a sexual encounter with your partner?
  9. 1Once or more a day

  10. When I have sex with my partner:
  11. 1I know I will be aroused and lubricated until we finish.
    2I am pretty sure I’ll be aroused and lubricated for the whole time and if not, we use lubricant and I’m fine.
    3Sometimes I will not stay aroused and lubricated for the whole time and lubricants don’t always help.
    4I am anxious because often I don’t “stay” turned on during sex.
    5I never get aroused, wet, tingly or excited.

  12. Over the past 4 weeks when I had sex (intercourse or other physical sexual stimulation with a hand, mouth or vibrator) I had orgasms:

  21. During the last month when your partner approached you for a sexual encounter, how easy or difficult was it to respond positively?

  25. Over the past 4 weeks how difficult was it to reach orgasm when you self-stimulated (with a hand or vibrator):

    5I do not self stimulate.

  27. During the last month, which would you say best describes your behavior?
  28. 1It's easy to say yes when my partner approaches me and sometimes I'll initiate sex with my partner.
    2I don't have spontaneous urges to have sex, but when my partner approaches me, it’s easy to say yes.
    3I stay in the shower (or on the computer) hoping my partner will fall asleep before I come to bed.
    4I really want to make my partner happy , but I'm rarely in the mood
    5I love my partner, but I just can't bring myself to have sex with him/her.
